Secondary bushing  inage attached, data below any coments?

Whilst the temperature level may not seem that high, I would be concerned about this. From the image it is difficult to tell exactly where the hottest point is, but it may be internal and could be some distance away. If this is the case, it will be a lot hotter. This, combined with the voltage would concern me very much. I would take an oil sample and get some DGA analysis ASAP and top up the oil.

I'd concur that this is probably an internal fault and an advanced one. While a single data point of dissolved gas analysis (DGA) will not be very useful, it is probably useful to get one now in case you can't get to repairs soon and need to trend oil changes over time. Reducing and stabilizing loads would also be important. You don't want to let this one go and may want to suggest the customer schedule repairs at the next opportunity (certainly prior to critical summer loads). Keep us posted!

UPDATE:

 Sorry for the long delay but the lead time on the new transformer and replacement took this long at least with reduced load it lasted long enough. After it was removed I took the top access cover off and could not see the connection I needed to probably because the oil was so dark smoky colored I could only see about 10" but it also took a week and a couple cleanings to get the burnt smell out of my jacket. To me the dark oil and the burnt smell physically confirms there was some kind of connection problem inside. Thanks for your help.
